Focal myositis: an unusual presentation of pediatric systemic lupus erythematosus

Abstract

Systemic lupus erythematosus may present in various forms, making it a diagnostic challenge. We report an unusual case of a pediatric patient who presented with focal myositis. Clinical case: An 11-year-old patient presents erythematous lesions on her right forearm that developed during one year, followed by a diffuse volume increase of the forearm. Cellulitis is suspected and due to lack of response to treatment, the patient is referred to rheumatology. Magnetic resonance imaging was performed showing an image compatible with myositis. A muscle biopsy of the lesion was requested but the patient returned five months later with fever, arthritis, weight loss, and claudication. Laboratory tests highlighted normocytic, normochromic anemia, ESR 71, ANA 1/640 homogeneous, Anti DNA(+)267 UdOM WHO/mL, C3 78 low, Ro, La and Sm positive. The biopsy was compatible with myositis. She started therapy with prednisone and methotrex­ate showing a favorable response. Conclusion: We did not find publications on pediatric lupus that debuted with focal myositis, making it relevant to report this case
